Would this be the ASQ contact or hotel after ASQ? Or can leave blank? Has anyone who’s entered into Thailand recently give some advise on this? Cheers from 🇬🇧

TLDR : Answer Summary
When filling out the form for entering Thailand, it's recommended to denote the address of where you'll be staying after your Alternative State Quarantine (ASQ). While many users report that they weren't asked for this information upon arrival, it's still best to include a hotel or intended address to avoid any potential issues. Some have successfully left this section blank, while others provided the ASQ hotel's name or their intended accommodation after ASQ. Overall, having this information ready is advised, even if it may not be strictly enforced at entry.

Although some people say it is not needed it is at least listed as requirement on the Washington DC embassy for the visa application and at entry into Thailand. I was asked for it at both the departing airport by the airline and by thai officials when I landed. As is the case with most requirements they may or may not ask for it. If they do and you don't have it then you may have issues.

I left this section blank

I filled this form out, however it was not needed. No one asked to see it when I arrived.
