There is form but some offices fill it in for you so you only have to give them your passport.

Yes get it from you Amphur office. Kor Ror 2. You need your IDs and some may require a copy of the marriage certificate.

Can I request a Re-Entry Permit during my 90 Day Check-In in Chiang Mai, and will my next check-in be 90 days after my return from Malaysia?

1 you can apply now.

2 if you leave before the 90 day it resets to 90 days after your return (Don't forget the TM30 needs to be done on your return. )
