Depending on where You do it you will need to fill the form and some photocopy of your passport, if there is a drive through and you have already proof of residence in your passport you need only your passport.

Exactly that- simplest of all thai forms.

I filled 1 in then made duplicates with date blank and photocopied for future use.

I did report in person last week asked about online- she said no come back here (Jomtien)
