Booked and paid for in advance - they wait for you at the gate as you disembark. Not aware of any that will offer on the spot service. Also the company I use won't accept a booking if you've ever had an overstay.

The company I use only cover BKK, DMK and HKT. Try Googling it though - makes the experience a lot less painful.
