"returned before the 90 day report was due"? Why? The 90 days are reset when you re-entered the country. Did they never tell you at immigration you were way too early?

no Thai bank account needed to apply for a non-immigrant O visa. This is only necessary when applying for your first EXTENSION.

What are the requirements for transferring 65,000 baht monthly into my Thai bank account for the visa application?

In Wise use "Funds for long term stay in Thailand" as the reason for the transfer. The code on the Thai bank side depends on the bank: Kasikorn labels it as FT (foreign transfer), no idea about the other banks.
