You're return or onward travel needs to be within 60 days. You could book a cheap flight out of Thailand within 60 days.

Anytime within 90 days of your departure. The visa is valid to use for 90 days from it's day of issue.

You can apply for one 30 days extention of stay. It's not a given although rarely refused, it's issued at the discretion of the Immigration Officer. Fee 1900 Thai Baht.
