So it seems doing the "new visa" rather than "extension" or "marriage" is the best option.

The biggest expat complainers about Thailand were miserable complainers back in their home countries too. No matter where they go, there they are complaining.

How do I correctly fill out the affidavit for a certificate of residency from the US Embassy in Thailand for my driver's license?

No need to bring a witness. Fill in part 3 prior to your appointment at the Embassy. Wait until there to complete 1 & 2 as to insure done correctly. Do NOT sign until with the notary. I recommend that you bring 2 copies. Beyond this I can provide no more insight.
