I am American, currently in the USA applying for a new O-A Retirement visa.

On the visa application, it says "In the case of submitting a bank statement, a letter of guarantee from the bank (an original copy) is required".

Does anyone know what this is?

TLDR : Answer Summary
The term 'letter of guarantee' refers to an official document from your bank affirming that you meet the financial requirements for the O-A Retirement visa application. It is typically required alongside your bank statements to demonstrate a stable income, usually exceeding 65,000 Baht per month. The letter may need to be printed on official bank letterhead, stamped, and signed. Various users commented that different banks have specific formats for this document and that all supporting documents should be properly notarized when applying.

I had the same issue when applying from the UK. I got my bank to print statements proving I had the income in excess of 65,000 Baht a month. They stamped each page and signed each stamp.

I then made 2 copies of all my supporting documents (bank statements, police check, medical....) and had the copies noterised. I applied in person at the Thai Embassy in London. If you are applying on line I would assume it is these noterised copies what you need to upload.

If you have a direct deposit that is what they are looking for.. We sent 6 months of just retirement deposits.. Tried sending just a normal bank statement and it kept coming back "letter of guarantee". After the bank printed out just monthly deposits we had them stamp and put initial it then we took it had had it notarized. They then accepted that... Good luck.. We had also sent 1099-R, LES, those were not acceptable..

I am doing it based on accumulated assets/savings not on income. I am wondering if I could have my statement notarized here in AZ...

the DC Embassy kept sending everything back so we started notarizing everything and then they approved it all so you never know

Normal. If you’re the bank print out your statements, they will include this type of letter with all the statement pages. At least I know Bangkok Bank does it.
