How do I correctly fill out the affidavit for a certificate of residency from the US Embassy in Thailand for my driver's license?

Hi, I am planning on applying for a Thai driver’s license and I am required to have a “certificate of residency”/ “residency statement” from my embassy (USA) ...the embassy website just says to fill out a blank affadavit form but offers no instructions ... can anyone that has done this before tell me how to fill this out correctly? Anything I should know like does it have to be printed or can be handwritten ? Specific ink color? Do I have to take a witness for the notary? Any documents I need to bring to the embassy? Any Immigration Office provides Certificate of Residence, either free or 50 Baht (in CM) and you get it in few weeks. If you need next day, then most charge an extra 500 Baht for the fast service. Very easy.

Just go to your nearest immigration office and ask for resident certificate just need to pay 100-300 baht but actually it's free. If you don't want to be questioned just pay them.

you can only do this if you have done at least one 90 day address report.

Not true. Can be done on a tourist visa thats been extended for covid (no 90 day report needed).

Print it, don't hand write it.

Just make a self identifing statement of who you are, name, dob, and passport number and then say you live at what address. Make sure it's full address including district and sub district. All in English.

No need to bring a witness. Fill in part 3 prior to your appointment at the Embassy. Wait until there to complete 1 & 2 as to insure done correctly. Do NOT sign until with the notary. I recommend that you bring 2 copies. Beyond this I can provide no more insight.
