Translated to thai, than MFA if i am not wrong. there is a translation service on the 2nd floor of MFA building. there is a time frame that the documents is valid for, so you have to be fast.

go to the nearest immigration (it square, near lak si train station, red line) if you are in bkk, with tm30 and passport, 1900b, bring a pen, photocopy your passport at the same place, there is 3 copy shops at the same floor.
