Don't muddy the waters with things like a Thai driver's license and embassy letter regarding your Thai address. Where you're staying in Thailand is probably the least of their concerns for a sufficient application. No need to submit anything other than your bank statement because it happens to be in Thailand. Ensure the employment certificate states you will not be doing any business with Thai clientele or entities, and authorized to work remotely in Thailand. Simply submit your contract, employment certificate, portfolio, brief cover letter explaining your remote work skill. Possibly apply in Vietnam. A million baht in the bank, get a real passport photo.

There is no student visa, there is an ED Visa. You would have to speak with the particular cooking institution to see if they have the certification and approval history to qualify you for an ED visa.

TM30 is needed for any services at your local immigration, it's the filed TM30 address that defines which local office serves you. She can easily create an account online and file your information at the address you're staying. You haven't encountered any TM30 pushback entering and exiting Thailand through passport control because they don't handle anything related to TM30.

I'll give you the short ugly truth. Even if you were to get in on a DTV stamp this time, you'd have to do this every 6 months for the next 5 years. The approved passport that the issued visa is tied to is missing, typically you would have had to carry both passports for 5 years.

Ultimately you're going to have to apply for a new DTV.
