Don't bother, they will make you fill in the exact same one again when you get there. Twice I've been sent back because of this (it was 100% identical)

3. Correct. Although it is HIGHLY advisable to get a private taxi from the consulate direct to udon airport due to time constraints. (The taxi will drive through the border and you will get your thai stamp quicker too) Pay no more than 1000 baht for the ride. No need to speak Thai or Lao.
Thanks Tod, in what circumstances would it take longer than 2 working days? I would not like it to take longer that. I will likely also use an agent if this will help.
