How do I complete the 90-day check-in at the DTV in Bangkok?

We did ours last week. Went to Division 1 with our passport, TM30 and they will give you a form to fill out. You line up (give yourself up to 2 hours if you get there at 830am) and they will stamp your 90 day check in. Cost nothing to do.
