 Not true. The Thai embassy in France and Switzerland don't issue visas to non-residents since ages.

 not instead. There is a field for uploading the wax card/certificate and another field to upload the cropped QR code.

All health insurances cover Covid. It's a disease like the others. The good news is that the Thai authorities don't ask it to be mentioned in the certificate since Nov 1st.
