Generic application for all visas. Not available for the OA visa. The OA is a multi-entry entry visa.

Yours is a Non Imm OA visa, the OP has a Non Imm O visa. Two different animals.

It shouldn't have been stamped used. It's a mistake by the Immigration Officer who stamped you in the first time you entered with it. A single entry Non imm O visa would've been stamped 'used'.
