1. Address in Thailand. I gave them a copy of my lease and put my personal telephone.

2. I would use your sole proprietorship info (I did). I also gave them a copy of my AoI (business registration). I also included a cover letter introducing myself and my company. Make sure you include something to the effect of - my clients understand work will be conducted remotely from Thailand.

I got mine in Laos btw.

You can keep your passport as is. When you do renew you can go to Immigration to have everything transferred to your new passport. I paid an agent 2000b to do it for me.

Just remember that you can’t travel if you have less than 6 mths remaining on your Passport.
