Echoing others comment. Attempt to do it yourself first. Join the DTV FB group and search for information there. Don’t get disappointed with rude remarks from others. Nice and helpful people also exist that will help you but exert effort by searching information in the FB group first and ask for clarifications.
I was told that you can avoid the fee if you withdraw using the bank app (Bangkok Bank) on your phone while in Pattaya. I have never tried it as I stay only within Bangkok.

For reference, I submitted my application online to Ho Chi Minh Thai Embassy on Monday afternoon October 21. Was released on 11:30PM Thursday October 24, a day before my flight from Danang back to Bangkok. I can relate. It was nerve wracking.
I don’t think a letter is needed. The Embassy does not need to know where the money came from. Not my personal experience, I read on some posts here (if you search with a keyword “500,000” or funds) people transfer the required amount from a brokerage account to their main account days before a monthly statement is issued so when they download the monthly statement the ending balance would show 500K baht. Personally, I submitted 3 monthly statements showing the required amount. Each embassy has their own interpretation of the requirement but to be safe, I would submit 3 monthly statements showing the required balance. Btw in my case I did not show the details, just a screenshot of the summary page. Good luck!
Each branch set their own rules. I was able to open a bank account with 90-day Non-O Visa at the Bangkok Bank located inside Emporium Suites. I brought with me Canadian passport, Canadian Driver's License, copy of Non-O Visa. Get off at Phrom Phong BTS and go to Emporium, not at Emquartier or EmSphere. Dress a bit nicely. You need a local SIM so get one before going there. You will need it to setup your banking app. They will help you there. You know you are at the right branch if you pass by Tiffany and Cartier stores. The branch is just around the corner.
When you arrive here you could take a health check up package in one of the hospitals or clinic. If results would show a chronic disease, there is a chance for you to apply for DTV soft power medical treatment. You can DM me if you have questions.
