I had exactly the same thing happen to me last week while in KL, i used the first QR code/Thai pass they sent and brought the second with me in my bag in case they asked. They never asked and i entered without any problems. I also loaded just my CDC card from when i got my J & J shot in Houston

My wife called all of them numbers yesterday and only the top one works. I did get my pass approved over night

I bought AXA and my circumstances changed and i could not travel. They wont refund if you cancel, they will only allow you to move dates. I have bought Tune also and they could not be more helpful and refunds are easy with them.

Use Tune not AXA. AXA will not give refunds if you need to cancel for what ever reason. Tune will, si use Tune

Stay away from AXA, they wont give you a refund if you need to cancel the policy. Tune do give refunds if you need to cancel anytime. With AXA you can only move dates.
