yes, if you're applying check this group for different embassies requirements from anecdotal experience (of course, could change). Some also take longer to process. On eVisa website you apply to the embassy where you're physically located (and have to show proof that your in that region)

the requirements are different from embassy to embassy. There are the very basic DTV requirements but some embassies request additional documents. Some request many more documents. E.G. the DTV requirement is to have financial proof of 500K baht. Some embassies require 6 months of bank statements as financial proof, some only 3 months, some only need proof that 500k baht equivalent was in the bank at the time of application.

Kuala Lumpur is notoriously difficult. HCMC and Hanoi seem to be the best/easiest to apply from.

no, every single page of passport is not standard. Biometric page is standard. Also the request for 6 months income slips is not standard. Simply 500k baht in the bank is standard.

I'm willing to bet this may be the KL embassy, who seem to have much higher requirements. HCMC and Hanoi are better (i.e. easier due to less requirements) embassies to apply from.

From what I've seen in the posts, the biggest "confusion" seems to be related to the shocking amount of native English speakers misunderstanding the definition of the word "within".

The flight you add in your application is your tentative or proposed travel. But there's no requirement to actually take that flight. Once you get the DTV you'll have 5 years to enter Thailand.
