The "Notice" is the paper you get once you've done your report. You can't have one unless you've done one before and it's only valid until you leave the country or you do your next one.

You don't get a re-entry with DTV.

You can a new 180 day stamp every time you enter. It doesn't matter how long you stayed inside or outside the country.

And did you just give them your TIN, or did they want a tax resident certificate too?

Immigration at the airport doesn't care about 90 day reports. You'll be totally fine.

The 90 day report is only checked at the immigration offices, and is technically not mandatory if you're only staying 180 days at a time and don't have to go to immigration for extensions.
