I'm Australian so obviously the US affidavit didn't apply to me. All I remember the amphur asking to state that I am not married to another woman. I married my Thai wife in Australia first, but then registered again in Thailand at the local amphur and had no problems.

I believe the local amphur ask you on the form if you are married to another person. It doesn’t ask you if you are single

If you were a resident in Aus for 35+ years, you don’t have to stay for 2 years. But need to apply in Aus. Check the Centrelink website
