Bangkok bank didnt say what the form is they need but showed me an example it is the translated copy stamped by your embassy

You go to your embassy and get a copy of your passport page stamped then get it translated to thai

Bangkok bank needs a copy of translated passport page stamped at your embassy then translated you also need a letter from immigration once you have a certificate of residency the letter is okay ing the bank to open a bank account I recently did this
