for the Thailand pass you must have insurance, a hotel confirmation including pcr tests, vaccine proof of required and your flight info, you can’t get approved without this, it has to for a specific date your are traveling on + 3 days for flight delay

No longer needed, you do need the T8, they give you one on the plane but it’s very small and hard to fill in, better to print an A4 one
