Phuket health form, as of today we have to fill out a health form when arive sandbox phuket, I hear on phuket news. Does anyone have a link for that form, so I can prepare it please.

What is the point of the T8 form nobody asked for it or even looked at it

On the T8 form where it asks you from and two and the seat number is that on the last flight into Thailand

Getting ready to fly to BKk tomorrow , filling out T8 form for you immigration, and it's asking flight number and seat number. since it's not a direct flight , I'm going from CMH , to Chicago, to DOH to BKK. Which flight number and seat number should I put down on the form ? And also since I have a layover on Qatar do I need put that down on the form asking what countries you been in last 2 weeks?
