 Some people used dodgy agents in Vietnam, Cambodia, Laos and Myanmar. Immigration have been suspicious of some DTVs issued from there for a while.

Highly unlikely to be asked to show it. Especially if you got your DTV from your home country and didn't spend a lot of time in Thailand previously.

Will I receive a 5-year visa if I enroll in a 6-month Muay Thai course for the Soft Power DTV visa?

Of course, you get a 5 year multiple entry visa. I got mine last August from London. Entered September and stayed for 6 months. Spent 3 days in Vietnam in March and got stamped back in no questions asked. Best visa I've ever had.
