For starters, bring both the old passport (do you still have it, even if it's void?) and the new one, along the visa (sticker or PDF print). They'll sort it out either at the airport or at an Immigration office.

they surely will know you're living abroad by seeing the transactions. I don't think they care whether is Spain, Thailand or America, but it's easy for them to determine you don't live in the country anymore.

It's not about the documents, but number of applicants. They need to vet not only DTV applications, but all sort of visa applications.

No 90-day report needed if you leave before day 90. You get 180 fresh days upon EACH entry.

Oh well, I see this is not what you were asking for. In my case, when I did my 90 days report I just used the last TM-30 I had at hand.

Yes, they (not you) must always file a TM-30 every time you check it at any hotel or accommodation, even if it's for one night only.

If you mean whether you'll become a tax resident after living 180 days in Thailand?: Yes

But it doesn't always mean you need to pay taxes here.
