You have to report every 90 days but ONLY if you stay up to 90 days CONTINUOUSLY in the Kingdom. For example I've been here for 14-15 months already, but needed to do the 90-day report only once so far. Every time you cross the border out of Thailand, that 90-day clock resets, no matter if you stay outside for one hour, one day or one month.

What the embassy said does make sense, tho. Especially, if you're Polish, you can enter visa free.

No, if you entered on visa exempt. Yes, if you actually got a TR (tourist) visa to enter. If you are from a country that is not on the visa exempt list, then you might have gotten a regular TR visa before. Did you remember applying and paying for one? If you didn't pay anything, then, no, it was a visa exempt.

You're not really asking for permission, it's just that the Embassy needs to verify you can do it. How can they know otherwise?

At what Embassy would you apply? They might think the headerless letter is sus, specially if you're applying outside of Japan.

There's no way to know at this stage. If your application was cancelled due to the wrong date, it means they didn't even review your other documents.
