I was completing the DTV application, but a question came up: “Have you ever applied for a Thai visa?”

I was there as a tourist, and later I requested a 30-day extension. Does that count as a visa?

No, if you entered on visa exempt. Yes, if you actually got a TR (tourist) visa to enter. If you are from a country that is not on the visa exempt list, then you might have gotten a regular TR visa before. Did you remember applying and paying for one? If you didn't pay anything, then, no, it was a visa exempt.
