Why not do the application online at home they sent the visa to your email. Takes 10 days and 120 dollars, don't use an agent do it yourself in khon kaen to get retirement visa and is easy. I just completed it cost 1000 baht

Bangkok bank didnt say what the form is they need but showed me an example it is the translated copy stamped by your embassy

You go to your embassy and get a copy of your passport page stamped then get it translated to thai
