For #3, I also have my own LLC that I used to get approved for the DTV visa. I presented the following documents: articles of incorporation (this was the LLC registration document that was created after I registered my LLC in the state of Florida) , a signed letter from my partner allowing me to work remotely, and my partnership agreement. If you don't have a partner, then last 2 documents are irrelevant for you.

No, I only registered my LLC this past June. They just need to see that it's a registered LLC.

my partner and I wrote up an agreement regarding the ownership percentages for each of us and different clauses that explain if we bring on a new partner, etc. It just gave more legitimacy for my LLC

I just got approved for the DTV out of Washington DC. For the employemwnt documents, I provided the articles of incorporation for my LLC, a signed letter from my partner indicating I am allowed to work remotely, and my partnership agreement. Hope this helps and good luck!
