I remember when Embassies could issue their own. I got one in Brussels and one in Singapore. It said FCO. Previously to that I think it said the office ie Glasgow, Durham etc
Good question. I put HMPO on the form internally at an Immigration Office just 2 weeks ago. However, with the DTV best to double check as some rejections in neighbouring countries due to this field.

Agents do have access in country ie with immigration. How much do they have with embassies and Consulates though. Yes, they might know tips and tricks plus where to apply. In my view those charging a few hundred USD for these services plus application are worth it for peace of mind for this with a complex history. The others charging exhorbaint amounts for a DTV where you do not have to attend any courses etc I would be wary of given past history.
I had to have a print out for an extension of a different visa 2 weeks ago in Chiang Mai. Like with most things it is often a different requirements at different offices plus the fact that many will not have completed a TDAC yet.
It is leaving the doors wide open for immigration if they want to be awkward with DTV holders. It is common knowledge they are not the greatest fans of the DTV.
I would avoid Poipet at the moment. It closed early yesterday due to the current Cambodia-Thailand border issues. Many were left stranded on either side. It opened late today with large queues.
