CapableCelery590 No. Applications apply through the eVisa website just like everyone else. The DTV is an MFA visa and Immigrstion have nothing to do with it until entry and extensions. I was being a bit facetious regarding agents and contacts. I think it is obvious some do with regard to the DTV.
Thailand420 I always had an agent or HR at work prepare all my documents. A few months ago I thiught I woukd do it myself this time. Chiang Mai are very helpful and indicated everything I would need and explained each one. All I really needed was a visit to my bank and they knew exactly what to provide. I have heard other offices not so easy though and people running backwards and forwards.
You need to fill out the TM47 which is the 90 Day report form. Hand that over with passport. I hand over TM30 but he seems to put it aside and never looked at it.

I very much doubt a small overstay will act against you with regards to a DTV application. There have been posts by people who were blacklisted for overstay for a year being successful with DTV. So not let it stop you applying.
