How can I submit the declaration form for e visa please give me information

I have applied for a TR 60 Visa in Canada but the Vancouver consulate has requesting that I send them the latest declaration form and I'm not sure which one it is I am attaching one that I found can somebody tell me if it is the correct form because the other form I found is quite different

Hello everyone! Just wanted to confirm you no longer need to mail in your declaration form with your other documents when applying for your visa? I went through the La Embassy and had too, but that was a month ago and it looks like they've updated the site and you don't have to anymore. If anyone who recently submitted paperwork could confirm that would be awesome.
