Looking to come to Bangkok for medical treatment for 3 months. What is the best way to go about a visa?

Tried different forums and YouTube vids. but am still not sure and confused with contradictory answers how I fill out the TDAC. If I am travelling from Australia to Bangkok and change over in Singapore ( 2hr change over time) from Australia to Singapore flight number XX. then when I change over and the flight number is XXX from Singapore, so different flight numbers, do I put country where you boarded Singapore or Australia?

I visit my wife in Thailand once or twice a year. Is it possible for me to get a Thai driving licence?
