Today I received my 90 day report from Nakhon Si Thammarat Immigration using the address above. It took 8 days from sending it to get it back. Cheers!

I posted on the Asean forums (Old Thai Visa Forum) and it seems like this address is the correct one.

I can confirm if I receive the receipt back to me.

Not sure how long this would usually take.
