You can try and have them write you a letter saying that “Mr X is a patient in our care and will have an ongoing treatment over the period of the next 12 months, receiving dental implants and related medical treatment on average of one appointment per month.” This should be more than sufficient IF the hospital/clinic agrees to this.
For the DTV visa application you don’t need to provide any hotel confirmation, you just enter a hotel name where you plan to stay. Put a major hotel name, done.
Was no problem whatsoever. I was curious about it and opened a Bangkok Bank account just to try it, easy peasy. Won’t be using it as I’m happy with KBank/UOB accounts that I opened ~ 10 years ago but it was worth it as a trial run (and I was bored out of my mind last week).
I’m not sure where you see that. All it asks is where your accommodation will be and you can enter whatever hotel name. No confirmation required for the DTV.
