If you had chance to apply other neighbouring countries, it should have been no risk and hassle free. However, some went out with DTV but no issue.

 My understanding is one doesn't need to stay 180 days or more. As long as you can prove that you are remotely working sometimes in Thailand.

I think that hospital could provide visa request letter to Royal Thai Embassy, Myanmar. You should produce cover letter is stating your treatment plans.
