requirements are copy of passport, visa page, tm30 and the certificate takes one week to issue. You don't need photos. I've just done it in Samui. And it was free of charge.

except it's not free, you pay for the NHS through National Insurance contributions. I agree everyone should have insurance 🙂

yes I was paying National Insurance until I left 5 years ago, started working at 16 and now I'm 43. Does that answer your question?

Good for you my experiences have been many and more unpleasant and inefficient than you describe.

speaking from my own personal experience, thank you for your comment! I would rather pay for care in Thai hospitals. I'm not discounting the many great medical staff that work for them, just overall the NHS service is not great. Long waiting lists/time to see GPs, not to mention the dreadful dentistry.

Lets not forget NHS is not worth paying for anyway.... Get insurance, end of story.
