You are more than welcome not to do your 90 day report, however be prepared to pay the fine for not doing so if you ever need to visit immigration in country

Try Mission Hospital Dr Nick, he is American but lived here for 20+ years. Always been great. Just book an appointment with him (about 700bt ) and tell him what you need I'm sure he would be happy to help

There is no salary requirement. Just the 500k in your account. Your financials will be fine
