Phnom Penh and Hong Kong have reportedly been cracking down on passports with too many visas. But there are many other embassy's you can go multiple times to before getting a warning like Laos or Vietnam. And the good thing with the warning stamp from those countries are they actually issue you the visa, but say its the last time, so you'll know to get a new passport by then

What I noticed about Hong Kong when I went there was it was the only consulate that had a sign posted about too many tourist visas, but it was unknown as to how many they look for before they choose to deny applicants. And I thought they only count back-to-back consulate visits. In your case, just 3 previous visas from other consulates, with no warnings and denied for 4th visa. I always thought that it was back to back visas from the same consulate, but if you go to different countries and get a visa from different consulates, they wouldn't count that visa from other countries against you

yes because if it is a real booking confirmation, it will show the price on the e-mail you get. I just showed the AirAsia email I got after I booked my flight, it also includes the receipt showing how much I paid, so why would this be an issue?

Always do your research before you go to an Embassy. Lots of Embassys these days DO require the 3 proof ofs, not just Bali such as Malaysia, Vietnam, etc.

1) Confirmation of itinerary out of Thailand

2) Bank statement at least 20,000฿

3) Hotel reservation/Lease agreement

Bali is one of the easiest consulates I have ever been to because no one is there, it takes 10 minutes total to be there and you're done. Just have the 3 documents required and there's nothing to worry about.

There is a big sign that says multiple tourist visas risk denial, so maybe if you have a lot of visas in your passport already they will ask questions but it was only my 2nd tourist visa in my passport so no questions asked for me. The only thing I gave them was copy of my passport, copy of HK entry slip, and 300HK for Visa.

One of the easiest and quickest consulates to get a Tourist Visa for me. The location is at someones house and there is never a line there, when I went I was the only one there to drop off my application and that took 10 minutes, pick up 2 days later and I was off to the airport.

Hong Kong is another consulate that does not require proof of funds, proof of onward travel and proof of hotel booking.

Drop off Monday, pick up Wednesday. A print out of your online banking balance is fine as long as it shows your name. You also need proof of onward travel within 60 days.

Since he canceled the extension he would have to leave before 31 July, since he stamped out of Thailand before then he should have a clean slate when he re-enters Thailand. If he stamps in on a Visa Exempt stamp he should start from the day he stamped in without any ties to when his extension is valid until since he had already departed the country. Now if he didn't cancel the extension and left, then yes he should still be using the re-entry permit date but I say since he canceled it, it should be null and void.

Because the border crossing at Nong Khai is one of the easiest borders to enter Thailand with. Just because you get a tourist visa doesn't guarantee you entry into Thailand especially if you fly into Don Mueng from Vientiane with a new Tourist visa, they can still deny your entry.
