Bali update. Just got a SETV from there, and it was fairly smooth. No problems at DMK either (I have a METV and a SETV and a good bunch of stamps from around the world in my passport.)
I went on Tuesday to get the form and try and confirm a few things (semi successful). Went again on Wednesday to submit and then picked up on Friday. During the first two visits I saw a grand total of three other tourists. EVERY one of which were turned away due to lacking documentation. The requirements are as listed on the Jakarta embassy page (which refuses to load at the moment) with a few twists.
First, I don't think they mention a copy of your main passport page. But bring it. They're not interested in your Indonesia stamp page.
Second, you need to show "proof" of payment of your onward flight from Thailand (which has to be withing 60 days!)
Third, a copy of lease is not good enough. That has to come with some additional documentation, like a signed copy of the owner id. Or house book or some bullshit. I opted for a (refundable) seven day hotel reservation. Which feels, well, beyond silly.
All in all a bit of a headache.
As a place to chillax and get a visa I really can't recommend it. Feel free to pm if you're interested in the more touristy aspects.
