You enter with a passport. You wont be coming in any capacity without a valid pasport with 6 months and depending on that passport, you may need a visa.

Is it better to get an OA visa in the USA or enter Thailand on a tourist visa and convert to OA visa?

Tourist visa is for tourists. Retirement visas are for living in thailand. What are you wanting to do?

A medical visa, if you are in a hospital and unable to leave from surgery. The hospital should be able to assist with that.
