I extended and all they asked for was passport, TM30 sheet and an application form. Nothing else....that was in Chiang Mai

Next year - as in 2022. No one can advise, rules may have changed by then. Google and embassy website give you most info you need now. Non-O with no insurance not likely to be possible ongoing. Assume you will need full insurance unless you go Elite Visa route.

....I knew someone who had an apartment spare. So was easy and legit. Technically any lease contract would be OK I would think.....

Confirmed apartment contract and perhaps evidence of payment works. That is what I used and was fine

2 documents. Basically A lab test result for the PCr test and a Dr certificate that you don't have covid (as per PCR result) and that you are fit to fly.

Emirates wanted both when I flew, but Thai immigration only wanted the PCT test certificate
