I wasn’t asked for any proof of residence. I just filled in the address of the gym when asked for ‘address in Thailand’ (the gym does offer bungalows for rent), but they never asked for any confirmation or other proof.

With a DTV you can enter Thailand on a one way ticket.

After DTV approval you have to enter Thailand within 90 days. After the first entry it doesn’t matter anymore how long you stay out.

No issue whatsoever. Pet documents are completely separate from (human) immigration documents (e.g. visas). For pets you deal with the Veterinary Authority and customs.

I’ve brought in my cat twice, both times on a TR60 visa. When I passed immigration, they didn’t even notice my cat and I didn’t mention her. After passing through immigration you go to the veterinary office (located in the baggage claim area) to do the ’immigration’ process for your pets.
