I got my DTV from Jakarta. I used my Thai address as current address; home country address as permanent; Thai landlord as local guarantor; date of last entry to Thailand as last visit. Passport valid in ALL countries.

Don't worry, if the staff finds anything wrong with your form, she will ask you to fill in a new form (available at the embassy).
