My spousal DTV visa was denied:

Your visa application has been cancelled. Wrong country of passport

I'm pretty sure it's because it asks issuing authority and I wrote "United States Department of State" as is on my passport and all they wanted was "United States".

It's annoying because the forms make a big deal about everything matching your passport and double check that everything on your passport is in the form exactly without any mistakes.

Anyway, I didn't know about this group until looking up this problem so at least I know where to look for issues when I apply next time. Better luck next time.

What do you fill in at "place of issue"? Most passports or maybe all of them are using the term "authority" for the issuing authority. My German passport lists the city and the mayor as the issuing authority. I googled other passports online, and many don't even list the city name, just the issuing authority. So what did you enter here? Just the country where the passport was issued, or did you copy exactly what's listed under "Authority" in your passport?

DTV rejected due to “Applicant provides incorrect information (country/territory issuing travel document) in the visa application.”

For reference, it’s a NZ passport issued in sydney, Australia.

The label on passport for issuing authority says DIA SYD which is what I put in the application.

I have no idea what should go in there.

Any ideas anyone?
