What form do I need from immigration to open a bank account in Thailand on an ED visa?

I just tried to open a bank account at bkk bank. They said I need a form from immigration. I am on an Ed visa. Does anyone have any idea what form this is and what I need/need to do to get it? Do I need a card or id from my school?
