What flight and seat numbers should I provide on the T8 form when transiting through multiple airports to Bangkok?

Getting ready to fly to BKk  tomorrow , filling out T8 form for you immigration, and it's asking flight number and seat number. since it's not a direct flight ,  I'm going from CMH , to Chicago, to DOH to BKK. Which flight number and seat number should I put down on the form ? And also since I have a layover on Qatar do I need put that down on the form asking what countries you been in last 2 weeks?  Thanks for any help

TLDR : Answer Summary

When filling out the T8 form for immigration into Bangkok, you should only include the flight number and seat number of your final leg arriving in Bangkok. Even though you have layovers in Chicago and Doha, only the last flight details are required. Additionally, you do not need to list transit countries like Doha when asked about countries visited in the last two weeks.

I wrote both off my flight details to be on the safe side...Bristol via Amsterdam to Bangkok...they turned away a passager in Amsterdam stopped him from boarding for having the wrong paperwork he was not a happy man

The flight and seat number you will be landing in Bangkok on. As far as the other I have never listed transit countries and never been questioned.
