Instead of the Non OA, consider applying for the Non O from within Thailand.

A much better option, in my opinion, and the opinion of others (above).

Before you come, purchase a one year (max) term of inexpensive travel insurance from World Nomads, Safety Wing or similar.

If your work permit was canceled 2 days ago, you have overstayed your visa, owe ฿1000 (2x500), and are now subject to arrest, detention, fine and repatriation. All at your own expense.

Leave Thailand today, and return on a visa exempt entry if you like, and are eligible.

Go to your local immigration offices and ask for a list of requirements for renewal.

It’s a handout they have available in English.

It’s ok to arrive 6 hours early for a international flight. Just be sure you clear immigration (not just security) before 11:59 pm.

I recommend you buy a lounge pass (if a first class lounge is open) after you clear customs so you can spend the 6 hours in relative comfort.

The US Embassy in BKK, as well as the US Consulate in Chiangmai do not issue residency certificates, nor have they ever in recent memory.

I’m not sure where or when you took the photo, but the list, unsurprisingly, appears ambiguous, and beyond what might be required by law… or by other banks, or even other branches.
