The cost is between 25,000 to 30,000 THB. The previous persons comment is completely false. When my wife and I arrived over 4 years ago we used an agent. Since then we have renewed our Visas by ourselves.
The only fee is for when you first get your visa. There is no charge for the 90 day report. Health insurance has to be valid for the number of days your visa is valid. If you only purchase for 90 days then immigration will only stamp you in for 90 days even though you have a one year visa.
I have an account with CIBC. I just inform my bank to transfer funds to my Thai bank. Just give the bank the Swift Code for your Thai bank along with the account number and address of your bank in Thailand. I have even had funds transferred in less than 24 hours

It is the airlines responsibility to insure you have a outward departure from the country you are going to. If immigration on arrival does not let you enter the country than it is the airline that is responsible to fly you back. The airline can be subject to fines or disciplinary actions if passengers are not screened properly upon checking. The only exception is if you have nationality or long term visa to allow you to stay.
